Please complete this form to request a new or replacement Crimson Card. Intial cards are issued at no cost for new students, faculty and staff. Replacement cards cost $20.

Your Crimson Card is your official Transylvania University identification as well as a multi-function card that can be used to access your meal plan and Dining Dollars, door access, and Crimson Cash. Report lost or stolen Crimson Cards to the Crimson Card Office immediately. Do not punch a hole in your card; physical damage to the card will deactivate it.

For initial cards or replacements with a new picture, please upload an image file (maximum file size 10MB). Provide a clear, recent photo of yourself, showing your entire head facing forward (don't angle or tilt your head or shoulders), and include space around your head so we can resize your photo if necessary. Photo is to be used for identification.

Acknowledgement

I acknowledge that my initial Crimson Card will be provided to me at no cost and that I will be charged a $20 fee for card replacement.
